This is where the NAIPC (National Aging in Place Council) and their website Age in Place come in particularly handy. The site is designed to help you make your own plan for Aging in Place and provide resources to support you. They have a wonderful planner (click here to download) for you to complete as a self assessment that can then be submitted to NAIPC for their evaluation and suggestions.
It's a holistic approach to aging in place including the key elements: Housing, Health & Wellness, Personal Finance, Transportation, Community and Social Interaction, Education and Entertainment.
